Community Health Needs Assessment & Implementation Plan

Every three years not-for-profit hospitals are required to conduct a Community Health Needs Assessment (CHNA) of the local populations they serve.

Our goal in conducting the assessment was to better understand the health status, behaviors, and needs of children and adolescents in South Florida so that we as a community – patients, families, Nicklaus Children's, and partner organizations – can optimally support their health and wellbeing. Implementation Plan Goals

Food & Nutrition Security

Ensure no family experiences hunger by advancing sustainable food security, promoting nutrition education, and supporting healthy eating in collaboration with community Collaborators so every child in South Florida can learn, grow and thrive.

  • Conduct screenings and referrals for food insecurity and other Social Drivers of Health
  • Utilize data to target efforts in neighborhoods with most need
  • Create and disseminate monthly blogs and content packages
  • Share nutritional education materials
  • Support awareness and enrollment for food benefit programs
  • Ensure materials are translated in coordination with community

Access to Information, Resources and Healthcare

South Florida families have the knowledge, resources, and supports to navigate and access reliable information, needed community services, and timely healthcare to enhance and maximize the health and well-being of their children

  • Coordinate health messaging through different mediums and languages
  • Promote resources though broad reach of The Children's Trust
  • Build and maintain public facing community resource website
  • Align with Florida Department of Health Community Health Improvement Plan
  • Support establishment of spaces for community to access telehealth and other resources
  • Design a program model for delivery of mobile health

Children's Mental Wellness

Children and youth in the community have access to preventative services and resources that support mental wellness, while also having timely access to qualified mental health professionals

  • Increase certifications in Youth Mental Health First Aid
  • Create clinical pathways for behavioral health patients
  • Implement awareness campaigns to reduce stigma and increase crisis supports
  • Build and sustain a network of school-based mental health professionals
  • Deliver expressive arts workshops to youth in the community
