1. Nicklaus Children's Hospital MOC Project Leader Application - This application is to be completed by the Physician who is the Project Leader and who desires to submit an initial application for a project for approval by the IRC with > 10 potential physicians participating in the project.
2. Nicklaus Children's Hospital MOC Project Leader Application - Simplified Version - This application is to be completed by the Physician who is the Project Leader and who desires to submit an initial application for a small project for approval by the IRC  or for a project with < 10 potential physicians participating in the project.
3. Nicklaus Children’s Hospital COVID-19-Related MOC Project Leader Application – Simplified Version - This application is to be completed by the Physician who is the Project Leader and who desires to submit an initial application for a small COVID-19-related project for approval by the IRC  or for a project with < 10 potential physicians participating in the project.
4. Nicklaus Children's Hospital MOC Individual Physician Project Participation Request - This request is for an individual physician who desires to join an approved project. This to be completed by the physician after discussion with the Project Leader of the approved project.
5. Nicklaus Children's Hospital MOC Project Progress Report - This progress report is to be completed by the Project Leader within 6 months of starting the Project. The Project Leader will be notified when the Progress Report is due.
6. Key Driver Diagram - Helpful road map to be used by the Project Leaders in planning the project
7. IRC MOC Project Approval Checklist Form - This is the checklist used by the IRC to gauge the merit of the project against the ABP requirements
